There's several threads related to language/region issues but I didn't find my particular issue mentioned.
My language/region are set to German/Germany. Many German web sites publish the same content in German and English. In those cases, I'd prefer to have German content results rated above their English translations (esp. as the former are often better-maintained). Kagi sometimes gets that wrong. An example query is "FU Berlin" (a Berlin university), where Kagi ranks https://www.fu-berlin.de/en above https://www.fu-berlin.de:
The bottom of the above screenshot also shows how the English-language Wikipedia entry is ranked above the German one (not shown here).
Another example query would be "Hertha BSC" (a Berlin soccer club), where Kagi prefers https://www.herthabsc.com/en over https://www.herthabsc.com:
The results for those queries don't appear to be stable for me, in some cases the German-language results are preferred as I'd expect. I think I've read somewhere that you're currently using Brave for maybe 10% of the queries, but I can obviously only guess whether that might be related.
I can easily imagine this problem being hard to solve, i.e. finding the balance for ranking local vs. international results being non-trivial. I do believe it to be a real problem though, even more so for non-geek users.
