This is a bit of a tricky one, as it sometimes happens, but I have not figured out when exactly.
Sometimes, Kagi shows me international results, despite displaying the selected region as "Germany", switching back to "International" and then back to "Germany" again, fixes it.

Firefox, International as region chosen in settings.
