I've been filling a bunch of feature requests and bug reports here, and every time I have problems with fitting description of the problem into the template. Currently, there are two edit fields both for the bug reports and feature suggestions, but each come with different prompts.
Let's start with a bug report. The first field asks to provide information on what happened, which is reasonable. The second field asks "What did you expect to happen? Describe the desired functionality" and that does not make much sense to me - this is often self-explanatory from the problem description in the first field.
As for the feature requests the two fields ask for description of the desired feature and details for how the feature will be used. Again, many features do not fall naturally into that template. Suppose someone wants to request some form of graphical redesign (say, place RSS on kagifeedback.org link at the bottom of the page instead at the top). What is the point of describing how such a feature is going to be used, if it is pretty obvious from the request description itself?
This division of templates into two edit fields feels contrived. I would very much welcome a single text field that allows me to provide a coherent description of the bug or requested feature. Of course, helper questions won't hurt.
Also, the fact that the contents of two fields is merged into one without any headers makes for poor reading if someone literally answers the questions.
