This form has a few accessibility problems.
First, there's a live region that causes NVDA to randomly shout out information as I type. For instance, I'll pause typing now, then try to type what NVDA says on its own as that live region changes...

It goes on, but you get the idea. It seems to be saying the results of the "related issues" list that was populated when I typed my title. Specifically, the last issue in the list. It does this every ten seconds, I believe.
Second, at the end of the form are two sections. One is a dropdown of some kind to choose where the issue happened ("this issue occurs on"), and the other is the "and involves" list. While these are both different control types, they share the same problem: there's no way for a screen reader user to know which option is selected. The first one, the "this issue occurs on" field, activating the menu works fine, as does choosing an option. The problem is that the button goes back to its collapsed state, its name not changing to show which option was chosen. Opening the menu again shows all the options, but the chosen one is not indicated.
For the "and involves" list of areas, this isn't a menu for some reason. Each potential problem area is clickable, but doesn't change to indicate whether it is selected. I can't tell if I can choose more than one, and I can't simply go through the options to tell which I selected.
Finally, the "what did you expect to happen" field has no label. It's clear what the field is for if I'm in browse mode and arrow to it, but if I simply tab to it, it has no indication of its purpose. I just hear that I'm in a multi-line text field.
These issues should be fixed. None prevent users from submitting feedback, but they are still irritating and should be addressed.
