The search results with a query that includes the "intitle" operator start with expected results, and are then followed by results that ignore the operator. As a result, there are more results than what should be expected.
For example, with the following query:
It should technically only retrieve one result, being the page of the part with reference "1910C9-5" on the the Makita (power tools manufacturer) website. Instead, this result does appear first, but is followed by numerous other results that don't have this specific part number in the title.
This is a specific example, but I've never found the "intitle" operator to behave as expected with other queries. Many of the results don't include the "intitle" content, or more accurately, only have the content appear somewhere within the page rather than the title.
