Sometimes when expanding inline image results (that is, image results shown on the Web tab), the domain settings button will be missing its icon:
This seems reproducible on any query that gives inline image results, though not every image has this issue. It seems to be based on the image in question, not just random.
Would expect every image to have the domain settings icon displayed properly:
