Irrelevant results keep appearing - OR operator is used automatically?

For some days now, I keep getting search results that are obviously not relevant to my query, and it seems that results are included which do not contain all the words I entered, as if the OR operator was added. For example:

The first result does not even mention pkcs11.so. If I put "No such file or directory" in quotes, the results are better and the irrelevant askubuntu result appears at position 5 instead of position one:

My queries are usually very specific. If there are no results, please tell me instead of showing results that are not relevant.

My expectation is that in every result, all words from my query are present.
