I regularly come across results that have previews and everything in the Kagi results but are 404s. It would be nice to be able to report these or trigger a recrawl upstream somehow. I don't recall this being a problem on other engines.
I imagine a user could click the adjustments icon next to a result and request a rescan or dead link recheck.
